Job Requirements
- Graduate of an accredited Practical Nursing program.
- Previous Long-Term Care (LTC), Skilled Nursing Facility (SNF), Rehabilitation, or related nursing experience preferred.
- Current Licensed Practical Nurse (LPN) license in the state of practice.
- Current Basic Life Support (BLS) certification preferred or required according to facility policy.
Essential Functions
- Provide direct nursing care to residents in accordance with individualized care plans.
- Administer prescribed medications, treatments, and therapeutic interventions safely and accurately.
- Monitor, assess, and document resident conditions, reporting significant changes to the Registered Nurse or physician.
- Assist with admissions, transfers, and discharges.
- Perform wound care, catheter care, tube feeding assistance, and other nursing procedures within scope of practice.
- Collaborate with physicians, Registered Nurses, Certified Nursing Assistants (CNAs), therapists, and other members of the interdisciplinary healthcare team.
- Maintain accurate and timely electronic medical record documentation.
- Educate residents and families regarding medications, treatments, and healthcare needs.
- Follow infection prevention, patient safety, HIPAA, and facility policies.
- Respond appropriately to emergencies and changes in resident condition.
- Supervise and delegate appropriate tasks to nursing assistants as permitted by state regulations.
- Perform other job-related duties as assigned.
