Essential Functions

  • Affirms and coordinates patients' care with the interdisciplinary team.
  • Documents patient conditions and nursing needs accurately and promptly in the patient's EHR.
  • Communicates with clinical leaders and Physicians regarding pertinent observations and reactions.
  • Assigns work to RNs, LPN/LVNs, and Nurse Aides as directed.
  • Serves as a supervisor, director, and evaluator for RNs, LPN/LVNs, and Nurse Aides.
  • Reports staffing issues and needs to nursing leadership.
  • Provides feedback to clinical leadership on staff performance.
  • Coordinates admissions, transfers, and discharges for patients.
  • Maintains inventory and supplies on the unit in a clean and safe manner.
  • Supports Performance Improvement initiatives.
  • Adheres to professional codes of ethics and maintains patient privacy.
  • Provides direct care and assists with care as directed.
  • Initiates the SBAR process and collaborates with the team for assessment and recommendations.
  • Assists physicians and consultants with special tests or procedures.
  • Coordinates patient assessments and updates the plan of care as needed.
  • Communicates changes to other team members and patient family/significant other.
  • Administers medication following the standards of medication administration.
  • Assists with the rehabilitation and treatment of patients.
  • Treats patients and their families with respect and dignity.
  • Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.
  • Interacts professionally with patient/family and involves them in the formation of the plan of care.
  • Identifies and addresses psychosocial needs of patient and family; communicates with Social Services/Discharge Planner.
  • Interprets data about the patient's status to identify age-specific needs and provide care.
  • Performs all aspects of patient care in an environment that optimizes patient safety.

Qualifications

  • Graduate of an accredited school of nursing.
  • Valid LPN/LVN license in the state employed.
  • BLS Certification.
  • ALS Certification preferred.
  • IV Certification preferred.
  • 2-3 years’ experience in nursing.
  • One year of supervisory experience preferred.
