About the role

The LPN plays a crucial role in patient care at Jefferson Health, focusing on patient and family-centered care. They exercise sound nursing judgment, collaborate with patients and their families, and implement care plans based on individual needs and cultural beliefs.

Responsibilities

• Interacts with co-workers, patients, and their family/designated care partner(s) in a respectful manner.
• Partners with patients and their family/designated care partner(s) to identify learning needs and reinforce education.
• Ensures safe, compassionate, and coordinated care based on patient preferences and values.
• Communicates effectively with patients, families, and other healthcare team members regarding patient care.
• Evaluates and collects data as appropriate within the scope of practice.
• Works collaboratively with the Registered Nurse and physicians to ensure safe and quality care.
• Documents patient care in accordance with institutional policies and guidelines.
• Contributes to the monitoring and improvement of nursing quality outcomes.

Requirements

• Graduate of an accredited School of Practical Nursing.
• Knowledge of patient and family-centered care practices.
• Cultural awareness and sensitivity.
• Effective communication skills.
• Knowledge of patient evaluation and care.
• Ability to reinforce patient education.
• Understanding of medication administration and prescription knowledge.
• Discharge planning and evaluation and diagnosis knowledge.
• Health information documentation skills.

Qualifications

• BLS - Basic Life Support - American Heart Association.
• LPN - Licensed Practical Nurse_PA - State of Pennsylvania orLPN - Licensed Practical Nurse_NJ - State of New Jersey.
