About the Role

The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by nursing assistants. This role ensures the highest degree of quality care is maintained in accordance with current federal, state, and local standards, guidelines, and regulations.

Responsibilities

  • Direct the day-to-day functions of nursing assistants and support personnel.
  • Plan and coordinate shift services, programs, and activities.
  • Provide written and oral reports concerning shift activities.
  • Admit, transfer, and discharge residents as required.
  • Complete and file recordkeeping forms/charts for resident admission, transfer, and/or discharge.
  • Receive and transcribe physician telephone orders.
  • Chart nurse’s notes reflecting care provided and resident responses.
  • Complete accident/incident reports and submit to the Director as required.
  • Perform routine charting duties in accordance with established policies.
  • Prepare and administer medications as ordered by the physician.
  • Maintain a good working rapport with inter-departmental personnel to ensure nursing services meet resident needs.
  • Create and maintain a warm, calm, and positive environment for residents.
  • Receive and give nursing reports at the start and end of shifts.
  • Make rounds with physicians as necessary.
  • Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, range of motion exercises, and post-mortem care.
  • Monitor assigned personnel to ensure compliance with safety regulations.
  • Maintain confidentiality of resident care information, including protected health information.
  • Ensure all nursing care is provided in privacy and personnel knock before entering resident rooms.
  • Perform other duties as assigned.

Requirements

  • Current unencumbered state Licensed Practical Nurse (LPN) licensure.
  • Ability to read, write, speak, and understand English.
  • Ability to make independent decisions when circumstances warrant.
  • Knowledge of nursing and medical practices, procedures, and relevant laws/regulations.
  • Current CPR Certification.
  • Experience in Long Term Care is preferred.

Physical Demands

  • Ability to move intermittently throughout the workday.
  • Ability to cope with mental and emotional stress associated with the position.
  • Must meet general health requirements, including a medical and physical examination.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of 5 feet and carry such weight a minimum distance of 50 feet.
  • May be required to assist in resident evacuation during emergencies.
