LPN - Continuous Care PM Shift

About the Role

As a Licensed Practical Nurse (LPN) in hospice care, you will provide compassionate support to patients and families during times of crisis, working under the supervision of a Registered Nurse (RN) to deliver bedside clinical care and enhance patient comfort and quality of life.

Responsibilities

• Deliver bedside clinical care under RN supervision
• Monitor patient conditions and report significant changes
• Follow physician orders to enhance patient comfort and quality of life
• Offer emotional and physical support to patients and families
• Collaborate with the RN case manager and healthcare teams

Qualifications

• Graduate of an approved nursing program
• Current Florida LPN license
• Minimum one year of acute care experience; hospice experience preferred

Additional Information
