LPN - Continuous Care - Nights - PRN

Schedule
- 1:1 Patient Ratio | (8:00 PM – 8:00 AM)
- Must include 2 weekend nights per month
Coverage Area
Manatee, Charlotte, DeSoto, and Sarasota counties.
Mileage Reimbursement provided.
About the Role
Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.
Responsibilities
- Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care.
- Reports significant changes in patient condition and family needs as needed.
- In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
Requirements
- Graduate of an approved nursing program with a current Florida LPN license.
- Minimum one year experience in an acute care setting, providing direct patient care.
- Previous experience as a hospice LPN preferred.
