LPN Continuous Care

About the role

As an LPN with the VITAS Intensive Comfort Care® (ICC) service, you will provide one-to-one crisis care for hospice patients whose health challenges require high-intensity, highly engaged clinical supervision for a short period of time. Your compassionate, responsive care will elevate patients' quality of life and alleviate caregiving pressures on their loved ones.

Responsibilities

• Care for hospice patients in their homes or assisted living/skilled nursing facilities.
• Work in shifts of eight to 12 hours, supporting patients in a comfortable, familiar setting.
• Visit the local program office once a week to meet with an interdisciplinary team including social workers, physicians, chaplains, and nurses.

Requirements

• Nursing license.
• At least one year of hospital nursing experience.
• Reliable transportation.
