LPN/CMA Care Coordinator

About the role

The LPN/CMA Care Coordinator is responsible for providing comprehensive care coordination services to patients in a healthcare setting. This includes managing patient care plans, ensuring timely medication administration, and collaborating with other healthcare professionals to enhance patient outcomes.

Responsibilities

• Develop and implement care plans based on patient needs and medical guidelines.
• Monitor patient progress and adjust care plans as necessary.
• Ensure accurate and timely medication administration and documentation.
• Communicate effectively with patients, families, and interdisciplinary teams to ensure clear understanding of care plans and expectations.
• Participate in regular meetings with healthcare providers to discuss patient progress and plan next steps.

Requirements

• Bachelor's degree in Nursing or Healthcare Administration.
• Current licensure as an LPN or CMA.
• At least 2 years of experience in a healthcare setting.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.

Pay
