About the role
The LPN/CMA Care Coordinator is responsible for providing comprehensive care coordination services to patients in a healthcare setting. This includes managing patient care plans, ensuring timely medication administration, and collaborating with other healthcare professionals to enhance patient outcomes.
Responsibilities
- Develop and implement care plans based on patient needs and medical guidelines.
- Monitor patient progress and adjust care plans as necessary.
- Ensure accurate and timely medication administration and documentation.
- Communicate effectively with patients, families, and interdisciplinary teams to ensure clear understanding of care plans and expectations.
- Participate in regular meetings with healthcare providers to discuss patient progress and plan next steps.
Requirements
- Bachelor's degree in Nursing or Healthcare Administration.
- Current licensure as an LPN or CMA.
- At least 2 years of experience in a healthcare setting.
- Strong communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.
