About the role
Phoenix Home Care and Hospice is hiring an LPN Clinical Manager to lead and support their Home Health team in Independence, MO.
Responsibilities
- Support and guide field nurses and caregivers
- Assist with care plans and clinical oversight
- Conduct supervisory and client visits
- Help ensure quality patient care and compliance
- Communicate with physicians, families, and the care team
- Support scheduling, training, and clinical problem solving
- Be a leader your team can rely on daily
Qualifications
- Active Missouri or Compact LPN license
- Home Health Experience
- PREFERRED: Strong organization and communication skills
- PREFERRED: Ability to multitask and lead in a fast-moving environment
- PREFERRED: Positive, team focused attitude
