About the Role

The clinical care coordinator is primarily responsible for coordinating all patient care and clinical support for team members and overall day-to-day operations.

Responsibilities

• Coordinating all patient care and clinical support for team members
• Managing overall day-to-day operations
• Handling intake processes
• Managing authorization and eligibility processes
• Document control management
• Scheduling patient care needs across various service lines
• Leading and directing the work of others to ensure quality patient care
• Demonstrating the company Mission, Vision, and Guiding Principles through behavior and job performance

Requirements

• Knowledge of a variety of home care concepts, practices, and procedures
• Knowledge of reimbursement guidelines
• Ability to accomplish company goals through clinical coordination
