LPN Charge Nurse - Evening and Night
Position Overview
As an LPN, you will provide direct nursing care to residents while collaborating with RNs, physicians, and the interdisciplinary care team. You’ll play a key role in supporting rehabilitation goals, ensuring comfort, safety, and dignity in a long-term care setting.
Key Responsibilities
- Administer medications and treatments as prescribed
- Monitor, document, and report changes in residents’ health status, including vital signs
- Auxiliary residents with activities of daily living (ADLs) as needed
- Collaborate with rehabilitation team members to support recovery goals
- Maintain accurate and timely documentation in compliance with facility policies and regulations
- Provide emotional support and education to residents and families
- Ensure a clean, safe environment by following infection control protocols
Qualifications
- Current and valid Licensed Practical Nurse (LPN) license
- Strong clinical skills, including medication administration and wound care
- Excellent communication and interpersonal skills
- Ability to work effectively in a team-oriented environment
- Caregiver-focused approach to care
- BLS/CPR certification required
