About the role

Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes.

Responsibilities

• Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
• Congratulate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
• Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.
• Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
• Serve as the CCHCS information specialist.
• Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.

Requirements

• Graduate from an accredited Practical Nursing Program
• Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
• One year of clinical experience in Homecare (preferred)

Qualifications

• Physical Requirements: Sitting, standing and walking for extended periods of time; Manual dexterity to perform work on a PC; Good visual acuity through normal or corrected vision; Ability to hear typical conversation levels through normal or corrected hearing; Medium Work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects; Physical Demand requirements in excess of those for Light Work
• Personal Protective Equipment: Follow standard precautions using personal protective equipment as required

Pay

Minimum hourly: $20.77 Maximum hourly: $31.68
