About the Role
The Care Nurse Supervisor (CNS) manages the successful operation of the Resident Care Department under the leadership of the Resident Care Director (RCD). The CNS is responsible for the oversight, coaching, and direct supervision of all resident care service providers in the community, including direct supervision of resident care positions, resident assessment, and management as defined by the RCD.
Responsibilities
- Demonstrates expertise in clinical skills and nursing judgment within their scope of practice.
- Identifies and assesses the clinical status of potential residents.
- Obtains the medical information release form from primary care physicians as needed.
- Develops a plan of care (in collaboration with the RCD) based on identified physical and psychological problems of the resident, including resident-specific goals and planned program interventions.
- Successfully implements the plan of care.
- Develops a service plan (in collaboration with the RCD) according to state regulations.
- Coordinates care needs with community providers via an effective case management process.
- Measures and accurately records vital signs (temperature, pulse, respiration, blood pressure, weight, and height) as per state regulations.
- Oversees ancillary health services as needed.
- Provides ongoing supervision of residents’ health-related activities, complies with health care instructions, and promptly seeks medical attention when needed.
- Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
- Conducts SAMM / MAR and RCA documentation audits bi-monthly.
- Conducts assessments/reassessments per Benchmark standards.
- Assesses risk in a proactive manner.
- Facilitates weekly resident tracking meetings in the absence of the RCD.
- Ensures resident information is communicated to staff through a report process and calendar (e.g., appointments, outings).
- Communicates and documents residents’ material needs (e.g., briefs, clothing, toiletries).
- Notifies RCD of incidents in a timely manner and accurately completes incident reports prior to the end of the working day.
- Observes and reports significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director per state regulations.
- Responds to emergencies and personal emergency response systems and knows when to call for backup.
- Ensures compliance with all resident care-related regulations (e.g., medication management).
- Demonstrates infection prevention and control, personal and resident safety, and reviews fire, safety, and elopement procedures.
- Ensures a visit occurs following an incident, accident, or injury and ensures physician and family notification per state regulations.
- Actively participates in the QA process per assignment.
- Directs and supervises the daily work assignments of Resident Care Assistants (RCA) and serves as the first point of contact for RCAs regarding assignment questions or concerns.
- Provides input into performance reviews and disciplinary actions as required.
- Participates in the hiring and termination processes as they relate to resident care associates, including sourcing, screening, and facilitating terminations.
- Conducts or supervises the scheduling of all RCA staff.
- Closely mentors, guides, and directs Lead Resident Care Assistants in all aspects of their job.

Qualifications
- Graduate of an approved L.P.N. program (per state requirements).
- Valid, unrestricted nursing license.
- Minimum 1 to 2 years of experience working as a nurse.
- Previous experience working with the elderly in a paid or volunteer position highly desirable.
