About the role
Vinfen is a nonprofit organization providing community-based services to individuals with various disabilities and health conditions. This position is located in a residential setting tailored to meet the needs of individuals with brain injuries, intellectual and developmental disabilities, or other complex health needs.
Responsibilities
- Nursing Care
- Develops and implements person-centered treatment care plans.
- Assists with activities of daily living, including showering, dressing, shaving, mouth care, foot care, diabetes care, wound care, GT/JT care, trach care, and nutritional support.
- Makes observations and performs individualized assessments with any change in status, providing skilled nursing interventions as necessary.
- Administers medication and treatments as ordered by the primary care provider.
- Provides transportation and accompanies individuals to medical, dental, or other appointments.
- Maintains nutritional needs and ensures timely ordering of pharmacological, medical, nutritional supplies, and medical equipment.
- Utilizes behavioral strategies and follows clinical plans as indicated.
- Supervision
- Supervises Certified Nursing Assistants (CNAs) during their shifts.
- Maintains accuracy and completeness of care provided, ensuring it aligns with ISP goals and addresses the whole person.
- Maintains compliance with quality control monitoring and ensures the program adheres to Vinfen policies and procedures.
- Trains CNAs on relevant nursing education and skills.
- Communication
- Maintains regular communication with Program Nurse Administrator, primary care providers, family members, and other significant relationships.
- Communicates effectively with other Vinfen employees, outside agencies, community agencies, and neighbors.
- Uses the emergency and on-call systems appropriately.
- Reports any changes in status to appropriate individuals, including the Program Director, primary care provider, family members, and other agencies.
- Documentation
- Documents nursing care and treatments according to Vinfen policy and procedure.
- Documents medication administration accurately and in accordance with Vinfen policy and procedure.
- Ensures all documentation meets regulatory standards and Vinfen policies.
- Environment
- Maintains a safe, clean, comfortable, and welcoming environment for individuals and families.
- Maintains compliance with regulatory requirements and promotes an inclusive environment.
- Maintains medical equipment and supplies, including wheelchairs, walkers, Hoyer lifts, hospital beds, and other supplies.
- Community Integration
- Involves individuals in community outings and events.
- Respects individual preferences and ensures they are included in community activities.
- Advocacy/Relationships
- Advocates for individuals served and collaborates with other Vinfen departments and community providers.
- Participates in treatment plans, ISP, and other planning meetings as needed.
- Maintains effective relationships with individuals, families, guardians, and neighbors.
- Professional Development
- Acts as a positive role model and actively participates in trainings and conferences.
- Adheres to organizational, strategic, participatory, and collaborative skills.
- Physical Effort
- Requires standing, walking, bending, kneeling, stooping, crawling, climbing, and operating computers and office equipment.
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift.
- Driving Requirements
- Driving is a requirement for this position, using either a Vinfen van or personal vehicle.
- A valid driver’s license is required and must be presented at the time of employment.
- Qualifications
- Typical Requirements: One to two years supervised practical experience, including internships, in a clinical or rehabilitation setting; experience working with individuals with developmental and/or psychiatric disabilities and medical illnesses; preference given to bi-lingual/bi-cultural applicants and applicants with lived experience.
- Preferred /Required Education: High school diploma or equivalent; Associate’s degree preferred.
- Driving Requirements: Must be at least 21 years old, have maintained a valid US driver’s license for at least one year, and pass a driver’s screening background check.
- One to two years supervised practical experience, including internships, in a clinical or rehabilitation setting helpful, but not required.
- Experience working with individuals with brain injury preferred.
- Strong commitment to the rights and abilities of individuals with brain injury who have complex health issues.
- Knowledge of LPN practice standards and intervention techniques.
- Sensitivity to cultural, religious, ethnic, age, gender, and disability issues.
- Skills and competency to develop supportive, trusting relationships with clients served.
- Knowledge of teaching and learning modalities.
- Ability to supervise and mentor CNAs.
- Knowledge of evidence-based practices and commitment to adherence to such practices.
- Knowledge and use of different communication and learning styles and supervisory techniques.
- Knowledge of organizational, strategic, participatory, collaborative skills.
- Knowledge of formal and informal assessment practices.
- Ability to make independent judgments and decisions.
- Ability to work in a professional and confidential capacity.
- Knowledge of personal computer applications and equipment.
- Knowledge of documentation standards requirements.
- Knowledge of proper body mechanics.
- Knowledge of available community health and mental health services.
