St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

Responsibilities

• Accurate assessment of each patient’s status, problems and needs.
• Collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an ongoing basis.
• Provides PRN visits on patients who call the answering service.
• Provides crisis care visits as assigned.
• Provides PRN visits as requested by PCC or MD.
• Provides care for patients for the entire County.
• Works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.

Requirements

• Current and active Florida LPN license required.
• Hospice, home health, palliative care, or related experience preferred.

Pay

$1,500 Bonus

Monday‑Friday: 8 am‑4:30 pm
