About the role

Traditional Home Health Care Pediatric Division is seeking a Licensed Practical Nurse (LPN) to provide one‑on‑one care in home and school settings for pediatric and adult patients in Wayne County, Lake Ariel, PA. The position offers flexible scheduling, competitive pay, and the opportunity to make a meaningful difference in patients’ lives.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Pay

$28‑$32 per hour

Schedule

• School accompaniment: 7:30am‑4:30pm, Wallenpaupack school district
• Saturday & Sunday: 7am‑11pm (shifts can be split)
• Monday, Thursday, Friday: 6pm‑6am
• LPN or HHA school hours: 7am‑4pm, home hours: 9pm‑7am
