Immediate Shift Opening: 2 nights per week, 8:30pm-5:30am (Mondays preferred). Temporary Need: Monday through Friday 5:30am-4:30pm.

About the role

Traditional Home Health Care (Pediatric & Adult Skilled) is a leader in the healthcare field, providing exceptional skilled nursing and aide services. We’re seeking a compassionate Licensed Practical Nurse (LPN) to deliver high-quality, one-on-one care to pediatric and adult patients in the comfort and safety of their own homes.

Responsibilities

• One-to-one patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care
• Administer prescribed medication, treatments, and therapies
• Perform patient assessments and coordinate care
• Report any changes in client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
