About the role

This full-time Licensed Practical Nurse (LPN) position supports clinical operations and delivers compassionate, patient-centered care at The HealthCare Connection’s Forest Park Health Center location in Lincoln Heights, OH.

Responsibilities

• Assist providers with direct patient care and clinical procedures.
• Conduct patient interviews and accurately document medical information in the Electronic Health Record (EHR).
• Perform routine screenings and tests as ordered by the provider.
• Handle telephone inquiries, provide clinical advice within scope, and conduct follow-up as needed.
• Ensure exam rooms are properly stocked, sanitized, and equipped.
• Coordinate referrals and pre-authorizations, and communicate next steps to patients.
• Maintain patient confidentiality and comply with all HIPAA regulations.

Qualifications

• Graduate of an accredited Licensed Practical Nursing (LPN) program.
• Current LPN license in the State of Ohio.
• Active CPR certification.
• Preferred: Minimum of one (1) year of LPN experience in a community health, hospital, or private practice setting.
• Strong communication and patient service skills.
• Ability to work effectively in a team-based, fast-paced environment.
• Familiarity with EHR systems is a plus.
