About the Role
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will play a key role in delivering direct nursing care, monitoring patient health, and collaborating with interdisciplinary staff to ensure the highest standards of care.
Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse (RN) or physician
- Administer medications and treatments as prescribed
- Monitor and record patient vital signs and conditions
- Assist with daily living activities and ensure resident comfort
- Maintain accurate and timely documentation of patient care
- Communicate effectively with residents, families, and healthcare team members
- Respond promptly to changes in patient conditions or emergencies
- Ensure compliance with all healthcare regulations and facility policies
Requirements
- Current, active LPN license in the state of employment
- CPR/BLS certification (or willingness to obtain upon hire)
- Previous experience in a skilled nursing or long-term care setting preferred
- Strong clinical, organizational, and communication skills
- Compassionate, patient-focused approach to care
- Ability to work collaboratively in a team environment
