About the Role
Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.
Responsibilities
- Maintains communications with Healthcare Providers regarding recent changes or requests for orders.
- Ensures that the community follows all federal, state, and local laws and regulations pertaining to clinical care.
- Collects and reports occurrences, and assists in developing or implementing improvement plans as necessary.
- Partners with the Administrator and other team members to maintain resident and team member health safety.
- Partners with a pharmacy consultant to provide optimal pharmaceutical services to residents.
- Responsible for infection control practices in the community.
- Oversees the safe and efficient distribution and administration of medications, injections, nebulizers, and moderate wound care per state regulations, stepping in when the floor nurse is unavailable.
- Identifies ongoing needs and services of residents through the evaluation and Service Plan process, including documentation for residents with changes in health care status.
- Completes any assigned evaluations.
- Partners with other department coordinators to identify, review, and discuss potential changes in service or medication levels, updating the resident’s service plan as required by state regulations.
- Maintains communications with residents’ families and/or responsible parties regarding changes in care or health concerns.
- Performs other duties as assigned.
Supervisory Responsibilities
- Provides supervision to Certified Nursing Assistants and others as assigned to ensure the delivery of consistently high-quality care to residents.
- Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Requirements
- Maintains a current state license as a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) per state regulations.
- Experience in assisted living, home health, or long-term care industries.
- Two (2) years of experience as a Licensed Practical Nurse preferred for overnight shifts.
- Must demonstrate competence in assessment skills, injections/medication administration, follow-up, and triage.
