About the Role

Loudonville Assisted Living residence is a well-respected, third-generation family-owned and operated business that has proudly served our senior community in upstate New York for over 50 years. We are seeking a Licensed Practical Nurse (LPN) to supervise residents and staff, ensuring safety, care, and compliance with state regulations.

Responsibilities

  • Assist in coordinating and managing residents' health needs with primary physicians, residents, care providers, and family members.
  • Supervise the intake of new medical/medication orders, distribution, and discontinuing of residents' medications.
  • Ensure accurate and appropriate charting in resident files.
  • Implement and ensure continued compliance with policies and procedures.
  • Oversee and direct duties of staff, including Medication Techs, Home Health Aides, PCAs, and CNAs.
  • Maintain knowledge of care needs for all residents under the enhanced license.
  • Meet customer service standards with a focus on resident satisfaction.
  • Stay knowledgeable of State Regulations (DOH).
  • Perform case management tasks as necessary.
  • Monitor care needs and report changes in condition to the Program Manager.
  • Participate in orientation, training, and retraining of staff/new hires.
  • Monitor staff to ensure assignments and duties are completed.
  • Identify staff training needs and assist with scheduling as needed.
  • Assist resident care staff in providing care and recognizing illness.
  • Maintain First Aid/CPR Certification.
  • All other duties as assigned.

Requirements

  • Licensed LPN in good standing with the New York State Licensing Authority.
  • Previous experience in care for seniors preferred.
  • Case management experience.
  • Possess a valid driver’s license.

Physical Demands

  • Moderate to heavy physical effort (up to 50 pounds) related to lifting, carrying, pushing, and pulling.
  • Must routinely lift, carry, push, or pull a minimum of 25 pounds.
  • Extended periods of walking or standing (on feet).
  • Ability to cope with the physical and emotional stress of the position.
  • Correctable eyesight to 20/40.
  • Correctable hearing must be clear and acceptable at 5 feet.
  • High-risk exposure to bloodborne pathogens.

Schedule

This is a full-time hourly position with a 5-day on, 2-day off rotating schedule, including every other weekend.
