About the role
The primary purpose of the Memory Care Center LPN is to maintain quality resident care in accordance with the Communities Policy and Procedure and federal, state, and local laws, regulations and guidelines while maintaining the resident’s highest quality of life.
Responsibilities
- Maintain superior supervision for staff assigned to your shift.
- Provide quality nursing care to residents in an environment that promotes resident rights, dignity, individuality and freedom of choice.
- Ability to follow, supervise and evaluate compliance of resident wishes in regards to resident’s rights and intervene when necessary.
- If leaving the community for any reason and you are in charge of the narcotics, you must count out and sign off with another nurse or CMT before leaving.
- Conduct resident rounds daily, report problems to DON.
- Identify resident problems and emergency situations and initiate immediate “lifesaving” measures in the absence of a physician.
- Administer and document medications and treatments per the physician’s order and accurately record all care provided.
- Maintain confidentiality of all pertinent resident information.
- Ensure resident concerns and/or grievances are reported immediately to the charge nurse.
- Be able to recognize a change of condition and report on resident’s condition to the charge nurse.
- Promote positive/open communication between team members, resident’s, visitors and other departments.
- Reorder medications from pharmacist and return all unused medications per facility policies and procedures as well as federal, state, and local laws, regulations and guidelines.
- Obtain vital signs as ordered/needed and record appropriately per facilities policies and procedures.
- Document all medication errors and report to the charge nurse and/or Director of Nursing.
- Absorb and assist in job duties as needed to meet resident’s needs.
- Report all incidents/accidents promptly and complete the required paperwork.
- Maintain a professional image at all times.
Essential Knowledge, Skills and Abilities
- Maintain Confidentiality of all Residents information and promote Resident’s right to choose.
- Graduate of state approved school of nursing and currently licensed with the state board of Nursing Registration and Education.
- Maintain appropriate appearance and personal hygiene.
- No inclusion on Employee Disqualification List.
- Maintain appropriate appearance and personal hygiene.
- Must be able to cope with the mental and emotional stress of the position.
- Be Courteous, considerate and cooperative in dealing with all community personnel.
- Appropriate results on Drug Screen Urinalysis.
- Ability to communicate effectively & respectfully with staff, residents and visitors.
- Maintain a CPR license.
Minimum Education/Equivalent Experience Requirements
- High School diploma or Equivalent
- Graduate of a state approved school of nursing
- Licensed with the State Board of Nursing Registration and Education
