About the Role
You will follow the services outlined in each resident's service plan in a friendly and supportive manner, ensuring compliance with local rules, regulations, and policies. This role involves hands-on care, wellness tasks, and clinical support to meet residents' specific needs.
Responsibilities
- Communicate with the Director of Health and Wellness regarding resident needs to ensure consistent quality services.
- Participate in providing hands-on care and specific wellness tasks as required by residents' care plans.
- Review care plans to ensure they meet residents' specific levels of support as required by state regulations.
- Provide clinical support and training to the direct care team to meet continuing education requirements.
- Assist in the transcription and review of physicians' orders.
- Maintain certifications required by local and state licensing agencies, including ongoing in-service trainings, accreditations, coursework, and certifications.
- Assist the community with compliance and quality assurance, reporting identified issues to the Executive Director.
- Review Medication Administration Records (MARs) and Daily Reports to ensure appropriate clinical interventions for residents on alert or with acuity changes.
- Follow all local, state, and federal rules and regulations related to resident care, documentation, HIPAA, OSHA, and Blood Borne Pathogen Guidelines.
- Work safely, support colleagues in maintaining a safe environment, and correct unsafe actions; comply with Safety Committee Standards.
- Assist with workplace injury investigations, return-to-work processes, and communication with care providers and management when required.
- Foster teamwork, show initiative in overcoming obstacles, and take actions that support others beyond job responsibilities.
- Model professional behavior in all communications with residents, families, visitors, physicians, and clinical/non-clinical staff.
- Perform other duties as assigned or needed.
