LPN
Job Summary
The LPN must be a clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.
Compensation
- LPN - $28.00 – $32.48 base hourly wage, dependent on experiencePLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials
- LPN Weekend Warrior - $28.00 – $44.98 base hourly wage, dependent on experience
- Weekend Warrior Program – choose one of the following 3 levels:
- Select Level: base hourly wage +$5.00/hour (every other weekend)
- Pro Level: base hourly wage +$7.00/hour (3 weekends per month)
- Elite Level: base hourly wage +$10.00/hour (4 consecutive weekends; 5th weekend off)

Qualifications
- Required: Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
- CPR certified
- Able to meet physical and job-related requirements
- Must be English literate and possess excellent written and verbal communication skills
Key Responsibilities
- Provide safe, effective nursing care per care plans and physician orders
- Administer medications and treatments accurately
- Assess residents and report changes promptly
- Document care in the EMR
- Maintains infection control and safety compliance
- Completes all other duties as assigned
