About the role

Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Little Falls, MN consists of 2 cottages/suites and up to 26 residents. We provide the utmost quality of care with an emphasis on person-centered care planning in our home-like atmosphere.

Responsibilities

• Medication management & chart auditing
• Performing resident treatments as ordered per the care plan
• Observing, tending to and documenting residents' health needs
• Communicating with RN Director regarding relevant staff and resident information and/or concerns
• Reviewing appointments & setting up transportation for residents
• Processing MD orders completely and accurately
• Completing incident reports as needed
• Ensuring the staff is performing safe practices
• Executing tours for potential admits
• Reviewing messages and communicating with staff through R-Task

Requirements

• Graduate from approved LPN program
• Possess a valid Minnesota LPN License
• Minimum of 1 year experience
• Leadership ability
• Must be able to occasionally fill in aide shifts when needed, including weekends
• Maintaining confidentiality and professionalism at all times
• Must be able to work as a team member and maintain a positive demeanor
• Positive work attitude

Pay

$27 to $29 hourly, based on experience in assisted living.

Schedule

Part-time (0.6 FTE), non-exempt.
