Job Duties And Responsibilities
- Completes nursing tasks as directed by a registered nurse or medical provider.
- Administers medications to patients when appropriate and maintains accurate records.
- Participates as part of an interdisciplinary team coordinating patient care.
- Makes patient appointments and collects health information and vitals for medical provider.
- Educates patients on monitoring and managing symptoms, medication, and health conditions.
- Collects blood, urine, and other specimens as needed for laboratory testing.
- Administers injections, TB testing, and vaccinations as ordered by medical provider.
- Completes health and symptomology screenings to aid in patient care.
- Affords assistance in scheduling patient appointments as needed.
- Coordinates with other medical facilities to obtain medical records for treatment of patients.
- Implements appropriate nursing interventions and actions according to established plan of care within scope of practice.
- Administers basic first aid, when necessary.
- Accurately and timely documents patient information and interventions in the electronic health record.
- Affords assistance in nursing coverage at multiple clinic sites.
Qualifications
- Licensed in the State of Ohio as a Licensed Practical Nurse (LPN).
- Two years related experience.
- Current BLS/CPR or willing to obtain within 30 days of hire.
- Valid driver's license and reliable transportation (travel between sites required).
- Ability to work flexible hours, including early morning, weekends, or evenings if needed.
- Experience in substance use treatment, behavioral health, or related nursing field preferred.
- Knowledge of medication-assisted treatment (MAT), harm reduction, and trauma-informed care is preferred.
- Ability to collaborate with all members of the healthcare team.
- Commitment to providing nonjudgmental, empathetic care to individuals with substance use and behavioral health disorders.
- Effective verbal and written communication skills.
- Exceptional customer service skills.
- Must be able to work as a team member.
- Proficient with Microsoft 365 applications, including Office Suite, Outlook, and Teams.
- Adept at learning and navigating electronic health record systems.

About the Role
JOIN OUR TEAM of dedicated professionals in fostering hope and transforming lives. Community Health Alliance is looking for a nurse to provide care in an outpatient behavioral health clinic at locations in Hamilton, Middletown, and Franklin under the direction of a Registered Nurse, Certified Nurse Practitioner, or Physician.

Schedule For This Position Is As Follows: Tuesday 7am-3pm, Wednesday 5am-1pm, Thursday 8am-4pm, Friday 8am-4pm, Saturday 5am-10am
