Job Summary

You will provide compassionate nursing care to our residents, under the direction of a physician or Registered Nurse. Your responsibilities include assessing resident needs, delivering essential physical or psychosocial care, and maintaining accurate documentation.

Responsibilities

• Assess healthcare consumer data including psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors.
• Contribute to the RN initial assessment and perform re-assessment.
• Set measurable and achievable short and long-term goals based on the assessment by the Registered Nurse, collaborating with the healthcare consumer and the multidisciplinary team.
• Develop and modify the plan of care, implementing and/or modifying it using assessment data, clinical reasoning based on standards of care and evidence-based practice.
• Coordinate care, educate, and discharge planning, being accountable for the care directly, indirectly, or through delegation.
• Evaluate progress towards identified healthcare consumer goals using a systematic process, revising the plan of care as needed and involving the healthcare consumer and the multidisciplinary team.
• Document all phases of the nursing process in the healthcare consumer’s record.

Job Requirements

• Graduate of a LPN nursing program, upon hire.
• Licensed Practical Nurse (LPN), upon hire.
• Basic Life Support (BLS) - CPR, upon hire.
