About the Role
Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.
Schedule
Overnight 12-hour shifts
Pay
Personalized compensation packages, including top pay with night and weekend shift differential. Loyalty and referral bonuses available. Responsibilities
- Maintains communications with healthcare providers regarding recent changes or requests for orders.
- Ensures the community follows all federal, state, and local laws and regulations pertaining to clinical care.
- Collects and reports occurrences, and assists in developing or implementing improvement plans when necessary.
- Partners with the Administrator and other team members to maintain resident and team member health safety.
- Collaborates with the pharmacy consultant to provide optimal pharmaceutical services to residents.
- Oversees infection control practices in the community.
- Responsible for the safe and efficient distribution and administration of medications, injections, nebulizers, moderate wound care per state regulations, especially when floor nurses are unavailable.
- Identifies ongoing needs and services of residents through the evaluation and service plan process, including documentation for residents with changes in healthcare status.
- Completes any assigned evaluations.
- Partners with other department coordinators to review and discuss potential changes in service or medication levels, updating the resident’s service plan as required by state regulations.
- Maintains communications with residents' families and/or responsible parties regarding changes in care or health concerns.
- Other duties as assigned.
Supervisory Responsibilities
- Provides supervision to Certified Nursing Assistants and all direct care staff to ensure consistently high-quality care for residents.
- Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Requirements
- Maintains a current state license as a Licensed Practical Nurse (LPN) per state regulations.
- Experience in assisted living, home health, or long-term care industries.
- Two (2) years of experience as an LPN.
- Demonstrated competence in assessment skills, injections/medication administration, follow-up, charting, triage, communication, and effective team leadership/supervision.
