About the role
Local home health agency is seeking an experienced LPN to provide one-on-one services to patients in their homes.
Responsibilities
- Providing medical services to patients
- Completing in home assessments and intakes
- Assisting with admissions
- Completing 90 day follow-up care
- Ensuring the patient care plan is being followed
- Completing required notes and documentation
- Educating patients and families on care, treatments and answering questions
Requirements
- Must be a licensed LPN
- Experience in a home health care setting is required
