Education, Knowledge, Skills and Abilities

• Graduate of an approved School of Practical Nursing

Length and Type of Experience

• Minimum of 2-3 years clinical experience in an acute care setting
• Able to provide independent transportation

Licenses and Certifications

• Current Ohio LPN license
• Pharmacy card

Physical and Environmental Demands

• Eye/hand coordination and manual dexterity sufficient to operate a telephone, analyze and prepare written records, and provide hands-on care to clients
• Normal range of hearing to communicate with staff, clients, physicians, and other health care providers
• Ability to lift and carry 25 lbs.
• Traveling to patient's homes in a 60-mile radius in all kinds of weather
• Ability to access a wide variety of home structures
• Involves dealing with frequent changes, stressful situations, and need to problem solve
