LPN - ACT Team Nurse

About The Role

Clarvida is seeking an ACT Nurse (RN or LPN) to join their Assertive Community Treatment (ACT) Team serving adults with severe and persistent mental illness. In this role, you will be a critical part of a multidisciplinary team providing intensive, community-based services designed to support recovery, stability, and independence.

Responsibilities

• Provide nursing services as part of the ACT multidisciplinary team
• Meet with clients in homes, community settings, and occasionally the office or via phone/Zoom
• Conduct basic health assessments, including vitals and labs
• Support medication management and facilitate long-acting injectable medications
• Care for clients with severe and persistent mental illness and/or substance use disorders
• Participate in a rotating on-call schedule (approximately one week every 7–10 weeks)
• Provide client transportation using a company vehicle as needed
• Maintain accurate, timely, and compliant documentation

Requirements

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Louisiana
• Experience working with individuals with severe and persistent mental illness and/or substance use disorders
• Psychiatric nursing experience preferred
• Valid Driver’s License in good standing and current automobile insurance
