Responsibilities

• Provide direct patient care in the home
• Monitor patient condition and report changes
• Administer medications and treatments as ordered
• Support individualized care plans
• Document care and communicate with the healthcare team

Requirements

• Active LPN license (required)
• Current BLS certification (required)
• 1+ year of nursing experience (preferred)

Pay

$32-$35/hr weekly pay

Schedule

Evening shifts: Monday through Friday, 3:00 PM to 11:00 PM. Flexible scheduling available.
