Long term care CNA

Provide routine daily nursing care and services to assigned patients in accordance with their assessment, plan of care, and care plan, as directed by supervisors.

Qualifications

• High school diploma or equivalent
• Current Certified Nursing Assistant Certification issued by the state where services are provided or due within three (3) months of hire
• WV: Nurse's Aide Certification through the West Virginia Department of Health and Human Resources
• State criminal background check and Federal (if applicable), as required for regulated areas
• Certification in Basic Life Support within 30 days of hire date

Preferred Qualifications: One (1) year nursing assistant experience in long term care.

Responsibilities

• Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner per Policy/Procedures and ensure they are correct
• Measure and record temperatures, pulse, and respirations per Policy/Procedures
• Demonstrate knowledge and skills necessary to provide care based on physical, psychosocial, communication, safety, education level, and related criteria for adult and geriatric patients per established age-specific standards
• Demonstrate ability to use equipment according to manual: vital signs instrumentation, k-pad, scales, oxygen tanks, drainage systems/suction, SCDs, CPM machine, and Bladder Scanner
• Assist patients with daily hygiene (dental care, mouth care, bath, hair care, backrubs, nail care, and shaving)
• Assist patients with nail care, including cleaning and clipping (except for diabetic patients per policy/procedure)
• Turn and position bedfast patients every 2 hours and as needed
• Assist patients with bowel and/or bladder functions
• Assist patients in bladder maintenance and bladder or bowel retraining programs
• Answer call bells promptly per policy/procedure
• Assist with the care of the dying patient and provide postmortem care per Policy/Procedure
• Provide perineal and indwelling catheter care per Policy/Procedure
• Assist patients with activities of daily living (ADL) with and without adaptive equipment as per instructions of therapist or nurse
• Serve food trays and assist with feeding as indicated; record food and fluid intake
• Assist patients with identifying food arrangements if they have sight problems
• Serve between-meal and bedtime snacks
• Assist with lifting, turning, moving, positioning, and transporting patients into and out of beds, chairs, baths, wheelchairs, etc., utilizing proper body mechanics
• Report all allegations of patient abuse and/or misappropriation of patient property per federal requirements and policy/procedure
• Report all accidents and incidents observed on the shift they occur per Policy/Procedure
• Assist patients with specialized feeding techniques as per recommendations of the speech therapist
• Assist with ambulation, gait training, and CPM administration as instructed by the physical therapist
• Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist
• Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients who require more than one person assistance

Skills

• Ability to comprehend and perform oral and written instructions and procedures
• Effective reading and comprehension skills
• Strong communication skills, both written and verbal
• Math skills for calculation of medications

Physical Requirements

• Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling
• Ability to lift, move, and position patients of all weights, with varying amounts of assistance, on a regular and recurring basis
• Hearing within normal range (e.g., to assess breath sounds, bowel sounds, apical pulse, monitors)
• Visual acuity within normal range
• Ability to perform heavy work: exerting in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects

Working Environment
