Phaxis is seeking a licensed Social Worker with care management experience for a full-time, temp-to-perm position in Bronx, New York.
About the Role
This is an 8-week local contract assignment with the potential to transition into a permanent role. The position involves coordinating care for older adults and collaborating with interdisciplinary healthcare teams.
Responsibilities
- Coordinate and manage care plans for participants with complex healthcare needs
- Participate in interdisciplinary team (IDT) meetings and collaborate with healthcare professionals
- Review clinical documentation for quality and compliance
- Coach and support field staff to ensure high standards of care
- Coordinate services including home care, rehabilitation, nursing home placement, transportation, and other community resources
- Monitor participant progress and modify care plans as needed
- Collaborate with physicians, specialists, discharge planners, and other providers to ensure continuity of care
- Maintain accurate documentation in accordance with regulatory and organizational standards
- Assist with staff training, competencies, and quality improvement initiatives
Requirements
- Master's Degree in Social Work (MSW) from an accredited program
- Current, active New York State Social Work license
- Minimum of 2 years of care management or healthcare management experience
- Experience in home care, managed care, long-term care, hospital, or related healthcare setting (preferred)
- Experience working with the elderly or frail population (preferred)
- Strong communication, organizational, and documentation skills
- Supervisory or leadership experience is a plus

Schedule
Monday-Friday, 8:00 AM-5:00 PM (40 hours per week, no weekends)
