LanceSoft is seeking a Social Work Licensed Clinical Social Worker for a local contract job in Walnut Creek, California.
Responsibilities
- Provide clinical social work services to patients in outpatient pediatric clinics at UCSF Health Benioff Children's Hospital.
- Collaborate with multidisciplinary teams to support patient care and family dynamics.
- Assess and address psychosocial needs of patients and families.
- Develop and implement individualized care plans based on assessment findings.
- Coordinate referrals and community resources as needed.
- Utilize Epic EMR system for documentation and communication.
- Floated to other UCSF facilities as required.
- Teach hospital staff about social work practices and interventions.
Requirements
- Active California Licensed Clinical Social Worker (LCSW) license.
- Minimum of 2 years of LCSW experience.
- Pediatric social work experience.
- Outpatient clinic experience.
- Case management and care coordination experience.
- Experience with Epic EMR.
- Ability to float to other UCSF facilities as needed.
- Teaching hospital experience.
