Host Healthcare is seeking a local contract nurse RN Case Manager, Acute Care Case Management for a local contract nursing job in Oakland, California.
Responsibilities
- Manage patient care plans
- Coordinate care among various healthcare providers
- Assess patient needs and develop treatment plans
- Communicate with patients and families about their health status and treatment plans
- Ensure compliance with healthcare regulations and standards
Requirements
- Bachelor's degree in Nursing (RN)
- Current California RN license
- Experience in acute care case management preferred
- Strong communication and organizational skills
Qualifications
- Passing score on the NCLEX-RN
- Minimum of 1 year of acute care experience
