About the role

A physical therapist is needed to provide home health therapy services in College Park, Georgia.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans based on patient needs.
• Document progress and communicate with referring physicians and other healthcare providers.

Requirements

• Current license to practice as a physical therapist in Georgia.
• Experience in home health care setting preferred.
• Excellent communication and interpersonal skills.

Qualifications

• Master's degree in physical therapy from an accredited program.
• Passing score on the National Physical Therapy Examination (NPTE).

Skills

• Proficient in using electronic medical records systems.
• Ability to work independently and manage time effectively.
• Strong organizational and problem-solving skills.
