Local Contract PT Home Health - $51-55 per hour

About the role

Host Healthcare is seeking a local contract Physical Therapist for a 13-week assignment starting September 8, 2026, in Portland, Oregon.

Responsibilities

• Provide physical therapy services to patients under the supervision of a licensed physician.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans accordingly.
• Document patient care activities and progress in accordance with regulatory requirements.

Requirements

• Current license to practice physical therapy in the state of Oregon.
• At least one year of clinical experience in physical therapy.
• Bachelor's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).

Qualifications

• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Strong organizational and time management skills.
• Proficiency in using electronic health records (EHR).

Skills

• Knowledge of various physical therapy modalities.
• Ability to work flexible hours, including evenings and weekends.
• Experience working with diverse populations.
