Local Contract Physical Therapist - $65 per hour

About the role

Select Source International is seeking a local contract Physical Therapist for a 13‑week Home Health contract assignment in San Antonio, TX. The role requires a licensed Physical Therapist to provide skilled therapy services to patients in their homes within a service radius of under 40 miles, using HCHB EMR. The assignment starts 10/12/2026, lasts 13 weeks, with a guaranteed 40‑hour work week (Monday–Friday, 8:30 AM–4:30 PM), no weekends or on‑call duties.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes
• Develop individualized treatment plans based on patient needs and goals
• Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence
• Monitor patient progress and modify treatment plans as appropriate
• Complete timely and accurate clinical documentation in HCHB
• Coordinate care with physicians, nurses, therapists, and other members of the healthcare team
• Educate patients and caregivers on appropriate exercises, safety, mobility, and home programs
• Oversee and supervise PTA activities in accordance with applicable requirements
• Manage an assigned caseload while maintaining productivity and quality‑of‑care standards

Schedule

Monday–Friday, 8:30 AM–4:30 PM; 40 hours guaranteed per week; no weekends; no on‑call.
