About the role
Host Healthcare is seeking a local contract Physical Therapist for a 13-week assignment starting on August 17, 2026, in Mendota, Illinois.
Responsibilities
- Provide physical therapy services to patients under the supervision of a licensed physician.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patient progress and modify treatment plans accordingly.
- Document patient care activities and progress in accordance with regulatory requirements.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Requirements
- Current license to practice as a Physical Therapist in the state of Illinois.
- Experience in outpatient physical therapy settings preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Qualifications
- Master's degree in Physical Therapy from an accredited program.
- Passing scores on the National Physical Therapy Examination (NPTE).
- Current CPR certification.
