Responsibilities

• Meeting members in their homes and communities (you will be completing 10 assessments/week minimum)
• Performing assessments and identifying care needs
• Building member-centered care plans
• Coordinating services and resources
• Supporting members through healthcare transitions
• Connecting members to social support programs
• Promoting wellness and preventive care
• Improving health outcomes through ongoing engagement

Requirements

• Current Massachusetts license as a Registered Nurse (RN)
• Associate's Degree in Nursing
• Either: 3+ years of RN/Behavioral Health Clinician experience, or 1 year of RN/Behavioral Health experience plus 2 years working with complex populations
• Valid driver's license, reliable transportation, and auto insurance
