About the role

Pride Health is actively hiring a Physical Therapist (PT) to join a growing Home Health team. This is a 12-week local contract role starting 09/21/2026, with a weekly hour guarantee of 32 and a productivity expectation of 30 points. The position offers a shift schedule of 8-hour days, 40 hours per week, with no on-call or weekend requirements. Mileage is reimbursed between patient visits (from first patient to last patient).

Responsibilities

• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments.
• Develop individualized treatment plans to improve patients' functional mobility.
• Deliver evidence-based therapy interventions and monitor/document patient progress accurately in Homecare Homebase (HCHB)/PointCare.
• Collaborate with physicians and the interdisciplinary team.
• Educate patients and caregivers.
• Ensure compliance with Medicare, agency, and home health documentation standards across assigned coverage areas (Centralia and surrounding areas).

Requirements

• Active Physical Therapist (PT) license.
• At least 1 year of home health experience.
• Dress code: navy blue scrubs (navy blue top with khaki shorts).

Preferred Skills

• Additional home health experience will set a candidate apart.

Pay

Local pay range: $55.00 to $61.00/hour (W-2).
