Location: Granite City, IL Coverage Area: Madison & St. Clair Counties, IL Pay Rate: $61.00/hour W-2 Duration: 6 months, with potential for extension Schedule: Monday–Friday, 8:00 AM–5:00 PM Hours: 40 hours/week Productivity: 25 points/week On-Call/Weekend: None Mileage: Reimbursed between patient visitsPosition Summary:We are seeking a Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes throughout Madison and St. Clair Counties, Illinois. The PT will be responsible for patient evaluations, admissions, discharge visits, development of individualized plans of care, and supervision of PTA staff.Key Responsibilities:Conduct home health physical therapy admissions and evaluations.Perform discharge visits and assess patient progress toward established goals.Develop and implement individualized physical therapy plans of care.Provide skilled PT interventions in the patient's home.Monitor patient progress and modify the plan of care as needed.Supervise and provide clinical direction to Physical Therapist Assistants (PTAs).Communicate effectively with patients, families, physicians, and the interdisciplinary care team.Complete required clinical documentation accurately and timely.Maintain compliance with home health standards, policies, and regulations.Required Qualifications:Current Illinois Physical Therapist (PT) license — required.Illinois is not a compact state, so an active Illinois license is required.Home health experience is preferred.Ability to provide care independently in patients' homes.Preferred Qualifications:Previous experience with Home Care Home Base (HCHB).Strong clinical assessment, communication, and organizational skills.Experience developing and managing home health PT plans of care.Dress Code: Business casual; scrubs of any color are acceptable.Patient Care Setting: Home Health Clinical Specialty: Physical Therapist (PT)About PRIDE HealthPRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
