LMSW - Hospice - PRN

About the role

Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team. Perform services in accordance with the physician's orders and the established plan of care (POC).

Qualifications

• Master's degree from a school of social work accredited by the Council on Social Work Education
• Certified or licensed in the state where currently practicing
• At least one year of social work experience in a health care setting
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions
• CPR certification

• Alabama: BSW can work under an MSW; MSW preferred
• Alaska: BSW can work under an MSW; MSW preferred
• Arizona: Licensed BSW must work under a licensed MSW; LMSW preferred
• Arkansas: Must be an LMSW, at minimum
• Colorado: Must be a masters level LSW
• Connecticut: Must be an MSW
• Delaware: Must be an LMSW; BSW can work under an MSW; LMSW preferred
• Florida: No state licensure required, certification only; must provide masters degree transcript
• Georgia: Must be an LMSW, at minimum
• Idaho: BSW can work under an MSW; MSW preferred
• Illinois: BSW can work under an MSW; MSW preferred
• Indiana: BSW can work under an MSW; MSW preferred
• Kansas: LBSW must work under an LMSW; LMSW preferred
• Kentucky: LBSW must work under an LMSW; LMSW preferred
• Louisiana: Must be an LMSW
• Maryland: LBSW must work under a licensed clinical social worker; masters level LCSW preferred
• Massachusetts: LSW can work under an LCSW; LCSW preferred
• Mississippi: BSW can work under an MSW; MSW preferred
• Missouri: State licensure not required; BSW can work under an MSW; MSW preferred
• Montana: BSW can work under an MSW; MSW preferred
• North Carolina: CSW can work under a CMSW; CMSW preferred
• New Mexico: LBSW can work under an LMSW; LMSW preferred
• Nevada: LSW can work under an LMSW; LMSW preferred
• Ohio: BSW can work under an MSW; MSW preferred

Preferred qualifications

• Medicare home health or hospice experience
