About the Role
The Lifestyle Companion provides personalized support to individuals living with dementia and their unpaid caregivers, focusing on social engagement, household management, and lifestyle coordination. Working within a dyad model and collaborating with an interdisciplinary team, the role involves managing daily logistics, facilitating meaningful activities, and supporting family caregivers. The position emphasizes a proactive, relationship-driven approach to prevent crises, ensuring tailored services that enhance clients' quality of life.
Responsibilities
- Manage household tasks such as transportation, meal planning, and home organization.
- Coordinate in-home services, caregiver schedules, and household modifications.
- Engage clients in social, cognitive, and recreational activities to promote well-being.
- Support family caregivers emotionally and practically, reducing their stress.
- Maintain communication with the team, providing updates on client needs and progress.
Requirements
- Bachelor’s degree in Gerontology, Social Work, Psychology, or related field, or equivalent experience.
- At least 2 years of experience working with older adults, especially those with dementia.
- Strong organizational, interpersonal, and communication skills.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- Experience in lifestyle coordination, event planning, or dementia-specific interventions.
Work Environment
Community-based, flexible, involving home visits, community settings, and administrative planning. The role requires both independent work and collaboration within a team to support client and family well-being.
