Life Sharing Nurse

About the role

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non‑profit organization accredited by the Council on Quality & Leadership, with locations throughout the West Hills and South Hills of Allegheny County. The Life Sharing Nurse position is full‑time (40 hours/week) with a flexible schedule primarily during daylight hours, based in Robinson Township.

Responsibilities

• The Life Sharing Nurse serves as a member of the Life Sharing team reporting to the Director of Life Sharing and assists with the provision of health and wellness care to adults and children with intellectual disabilities and autism in typical and medically complex (MCC) life sharing programs.
• Assists the Life Sharing Coordinator with completing the Health Risk Screening Tool (HRST) for all people receiving life sharing support.
• Works collaboratively with the Life Sharing Medical Coordinator to coordinate services supporting medical, health, and wellness needs.
• Brings a positive, person‑centered care approach and works cooperatively with administrative and program staff.
• Requires travel to the homes of people supported for home visits and offers flexibility to telework from home on a Citizen Care device or work in the administrative office as scheduled with the Life Sharing Medical Coordinator.
• Demonstrates a working knowledge and understanding of personal outcomes for people supported, especially as they apply to health, wellness, and medical needs.
• Advocates for people supported to achieve their personal health and wellness goals through education and consultation.
• Assists the Medical Coordinator with implementation/integration of the HRST; ensures routine updates and proper documentation within the screening tool.
• Works with team members to include pertinent service considerations generated by the HRST in the person’s ISP.
• Develops and provides training related to medical topics or identified medical needs of people supported across all programs.
• Completes and maintains certification as an HRST rater and clinical reviewer; serves as the clinical reviewer for completed HRSTs within the life sharing programs.
• Demonstrates familiarity with the Office of Developmental Programs’ (ODP) Medication Administration Process for Life Sharing Partners
