Hattie Larlham is a not-for-profit disability service organization providing care and support to over 1,600 children and adults throughout Ohio. We enhance the quality of life for people with intellectual and developmental disabilities and their families through our commitment to comfort, joy, and achievement. Responsibilities
- Provide suggestions for programs and activities for people with disabilities.
- Implement programming as indicated in persons served Individual Habilitation Plan (IHP).
- Assist persons served in participating in community trips.
- Conduct day program and habilitation activities as part of a coordinated day program.
- Attend team meetings for persons served.
- Collects and reviews program summaries, and provides these summaries to the IHP Team.
- Cookordinate and track Direct Service Professional schedules to assure a safe and therapeutic staffing pattern.
- Maintain attendance records for persons served in program.
- Implement and provide complete, accurate, and appropriate documentation of programs and activities.
- Serve in ratio for community trips and active treatment when needed.
- Provide input for program and activities.
- Develop a working relationship and exchange of information with parent/guardians.
- Selectively shares persons served information with service providers and advocates according to the persons served personal goals.
Qualifications
- High school diploma or GED.
- CPR & First Aid certification - free training provided.
- At least one (1) year of direct care experience in I/DD.
- Experience or willingness to work with people with disabilities in the area of habilitation, and day programming.
- Able to operate a computer and have a general knowledge of Microsoft Word, Excel, PowerPoint, Outlook, and Internet Explorer software.
- Must have a valid Ohio driver's license and satisfactory driving record.
