About the role

As a Life Coach/Qualified Mental Health Professional you will provide quality Mental Health Skill-Building services to adults through training and support, enabling clients to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. You will train and reinforce functional skills and appropriate behaviors related to the client’s mental health and safety, activities of daily living, and use of community-based resources. This position reports directly to the Clinical Supervisor and serves urban and rural populations throughout Hampton Roads and surrounding areas.

Responsibilities

• Maintain a caseload and provide goal-directed Mental Health Skill-Building Services to adults in the following areas: functional skills and appropriate behaviors related to the individual’s health and safety, activities of daily living, use of community resources (including locating shelter/housing, financial and food assistance, etc.), assistance with medication management, and monitoring health, nutrition and physical condition.
• Provide mental health skill-building services tailored to each individual’s specific needs.
• Deliver supportive counseling and crisis intervention services as needed.
• Develop, maintain, and implement the client’s Individualized Service Plan.
• Develop and review quarterly and monthly client reports.
• Develop, implement, and review client discharge plans.
• Produce high-quality, punctual, and accurate documentation consistent with Virginia Department of Behavioral Health & Developmental Services licensure and agency policies; maintain thorough records of all services provided.
• Attend and participate in staff meetings and agency trainings.
• Coordinate care to ensure clients’ health challenges are addressed.
• Participate in individual and group supervisions and maintain ongoing communication with program staff and other treatment providers.
• Transport clients to appointments and related destinations.
• Render services primarily in community settings including hospitals, group homes, and residential settings.

Requirements

• Bachelor’s degree in Human Services or a related field from an accredited college or university, or current QMHP-A credential.

Pay

$17.00 – $22.00 per hour
