Duties, Functions and Responsibilities
- Provide dedicated and extensive life coaching, case management and community resource navigation to caseload of Project participants through in-person and virtual meetings for at least one year.
- Completes a social assessment, which includes personal interviews as well as collection of secondary information to complete an individualized plan for self-sufficiency and responsible fatherhood.
- Research, contact and build relationships with community resource agencies, businesses and institutions to connect and refer Project participants to meet their needs.
- Work directly with HomeGrown StL partners and FFSC Quality Assurance Manager to co-create and implement Project’s Life Coaching cohorts and Community Resource Partner presentations.
- Facilitate in-person Life Coaching cohorts featuring Community Resource Partner presentations over the course of eight weeks and two-hour Sessions.
- Prepare and present electronic progress reports and caseload summaries during biweekly Status meetings.
- Absorb and assist with Project outreach, recruitment and enrollment through in-person and virtual presentations.
- Attend mandatory all staff meetings, Project training sessions, and community and networking events.
Knowledge, Skills and Abilities
- Must be able to manage high-volume caseload of up to 20 participants.
- Must have effective verbal and written communications and interpersonal skills with ability to multi-task.
- Must be able to utilize agency and community resources to meet goals.
- Familiar with MS Office Products.
- Must be able to organize and present information as well as work independently.
- Must have strong case management and documentation skills.
- Must understand and practice the NASW code of ethics.
Educational and Experience Requirements
- AAS or BS degree in Human Services or related field or HS Diploma/GED plus 5 years of experience working with at risk populations.
- BSW, MSW preferred.
- Must have experience in working with at-risk populations and prior work with group facilitation.
- Must be able to work flexible schedule with some evenings.
- Possess a valid driver’s license and have a reliable, insured vehicle.

Pay Range
$46,000/year
