About the role
The Clinic Therapist, LICSW plays a crucial role in creating a continued care plan for patients at Herself Health clinics. This position is hybrid, combining time spent at the Crystal and Rosedale Clinics with remote work.
Responsibilities
- Counseling and Psychosocial Support: Providing counseling and support to patients and their families to help them cope with illness, treatment, and life changes.
- Engages with patients and their families to provide telephonic support as a member of the interdisciplinary team.
- Advocates for patients' needs and ensures they receive appropriate and timely care.
- Collaborates with healthcare providers, patients, and families to coordinate care plans addressing medical, social, and emotional needs.
- Conducts comprehensive assessments to identify patients' social, emotional, and environmental needs.
- Connects patients with community resources such as housing assistance, financial support, food programs, and mental health services.
- Provides health education and promotes healthy lifestyle choices.
- Ensures care is culturally sensitive and appropriate.
- Maintains and monitors patient outcomes to ensure care plans are effective and makes adjustments as needed.
- Advocates for policies that improve access to care and address social determinants of health.
- Adheres to ethical standards and principles in all interactions and interventions, ensuring confidentiality, informed consent, and respect for patient autonomy.
- Addresses barriers to medication adherence with patients.
- Addresses needs related to Social Determinants of Health (SDOH).
- Reinforces the importance of regular follow-up visits with the PCP.
Qualifications and Skills
- Active, unrestricted LICSW license in the State of MN required
- 2+ years of experience supporting patients in a counseling setting
- Experience in behavioral health
- Experience engaging with patients telephonically
- Knowledge of available community resources and programs in the Twin Cities
- Excellent verbal and written communication skills
- Proficient in the use of web-based technology and Microsoft Office applications such as Word, Excel, and PowerPoint
- Must be able to effectively work in a start-up environment with frequently changing priorities, deadlines, and workloads that can be variable
- Requires reliable internet and a home office set up conducive to discussions of sensitive health information
- Experience using and documenting within an EMR system
